Nut Tree Road runs parallel to I-80 on the east side of town; the office is near the Nut Tree Plaza shopping area. Look for Suite 230.

Where do I park?
Free surface parking is available at the building. Enter from Nut Tree Road; our office is in Suite 230.
Is the office accessible?
Yes. The building has accessible parking and entry. If you use a wheelchair or have other access needs, call ahead and we will make sure you are set up comfortably.

You can email to request a call back or ask a scheduling question. Please do not send health information by email; call us for anything about your care.
What if I need care outside your hours?
We are open Monday through Thursday, 8:00 am to 4:00 pm. For a life-threatening emergency call 911. Otherwise call our office; the voicemail explains how to reach care and we return calls the next business morning.
